It is never too early to inquire and plan. Seeking advance consultation empowers the patient and his/her family, in ensuring that they make informed decisions about difficult end of life care, before it becomes a concern.

Many patients may have pain and other serious symptoms as illness progresses. Hospice staff receive special training to care for all types of physical and emotional symptoms that cause pain, discomfort and distress.

Hospice staff work with the patient's physician to make sure that medication, therapies, and procedures are designed to achieve the goals outlined in the patient's care plan. The care plan is reviewed frequently to make sure any changes and new goals are in the plan.

No specific therapy is excluded from consideration in hospice/palliative care. An individual's needs must be continually assessed and treatment options should be explored and evaluated in the context of the individual's values and symptoms.

Durable Power Of Attorney For Health Ccare
Everyone has the right to plan and make their own health care decisions. A Durable Power of Attorney is an "advance directive" to care givers, ensuring that your wishes are carried out, in the event that you are too sick to execute your wishes.
Living Life Fully
Hospice affirms life and neither hastens nor postpones death. Hospice helps patients achieve physical and emotional comfort so that they can concentrate on living.

Patients are urged to stay active for as long as they're able-for example, to take part in activities they enjoy or to do something they've always wanted to do.

When someone has a terminal illness, his or her loved ones feel pain too.

Hospice programs also provide relief to caregivers, who may neglect their own needs.

Our Interdiciplinary Team Provides The Following Services:
Physicians, Skilled Nurses, Home Health Aides, Spiritual Counselors/Chaplains, Dieticians, Physical Therapists, Occupational Therapists, Speech Therapists, Pharmacists, Volunteers and Durable Medical Equipment Care that targets the physical, emotional, psychosocial and spiritual issues of our patients Supplying all necessary medications, equipment and medical supplies Coordinating the patient's short term care in the appropriate facilities, as necessary; and Providing the necessary counseling and bereavement care to the family and friends following end of life. Can my pain & symptoms be controlled at home?
Pain and other systems can usually be controlled in the patient's home. If a symptom becomes a problem, the hospice nurse can be reached 24 hours a day, 7 days a week. There have been great advances in pain and symptom control in the past few years. Most symptoms can be controlled without the use of injections or IV medication. The hospice nurse will assess your pain and symptom control at each visit. Hospice medical directors are always available to adjust medications. contact us.
