Our team is an interdisciplinary group, consisting of the patient's personal Physician, Medical Director, highly-trained and motivated Nurses, led by our Nursing Director, Home Health Aides, Social Workers, Dieticians, Chaplains and other Counselors, Trained Volunteers, Speech, Physical and Occupational Therapists, who work as a team dedicated to rendering excellent services.
Our focus is to provide the highest quality of individualized physical, emotional, psychological and spiritual care to our patients. We adopt a family oriented approach in developing a care plan that meets the need of our patients and their families by combining our strengths and resources for ultimate care.
We provide hospice/palliative care to a person who has a life limiting/terminal illness. Pain management to ensure that every moment in our patient's life is free of needless pain and discomfort.
Can my pain & symptoms be controlled at home?
Pain and other systems can usually be controlled in the patient's home. If a symptom becomes a problem, the hospice nurse can be reached 24 hours a day, 7 days a week. There have been great advances in pain and symptom control in the past few years. Most symptoms can be controlled without the use of injections or IV medication. The hospice nurse will assess your pain and symptom control at each visit. Hospice medical directors are always available to adjust medications. contact us.
