Delta Hospice provides Comforting Care through our team. We are proud to have professionals such as Hospice Physicians, Registered Nurses, Licensed Vocational Nurses, Certified Home Health Aides, Social Workers and Chaplains.
Co-certifies the terminal prognosis with the attending physician. Leads the interdisciplinary team in the development of a plan of care. Provides consultation to other physicians regarding care for the hospice.

Director of Patient Care Services
Coordinates and oversees all direct and indirect patient services provided by clinical staff members.
Assesses patient and family needs and develops the plan of care. Coordinates team visits and ensures implementation of approved plan of care. Coordinates care with attending physician, the primary caregiver and/or care center staff. Ensures symptom control and pain management. Provides patient/family teaching as needed.
Licensed Vocational Nurse
Assesses patient and family needs in accordance with the plan of care. Ensures symptom control and pain management. Provides patient/family teaching as needed.
Certified Home Health Aide
Provides direct personal care to patient and emotional support to patient and family. Reports identified needs to assigned case manager
Assesses the emotional, social, spiritual and financial needs of patient and family. Develops a plan of care to meet identified needs. Provides direct counseling or refers patients and families to appropriate community agencies. Provides bereavement support.
Assesses patient and family spiritual needs. Develops a plan of care to meet identified needs and provides direct counseling. Assists with memorial preparations and provides consultations to clergy in the community.
Provide companionship and support to patient and family. Provide needed non-medical services, letter writing, errands and respite time for family caregivers. Provide support at the time of death and during bereavement.
